How frequently should you format a floppy disk?

To answer it straight- it's up to you.
I think the reason you are using floppy disk is: you don't have usb port. If you do have, then consider using usb flash. (Win98 can read usb flash by adding driver).
In a link below it says: "...Floppies are only good for a year or so, and often less than that. You should never trust a floppy to carry the one-and-only copy of any document. They definitely cannot be relied upon to hold data over the long term."
The only reason/time you need to format a floppy disk is when you want to delete all the information on the disk.
Wow...floppy disk...1.44 MB only! You can buy a 1 GB (1024 MB) usb flash drive for around $10 these days.
